To determine the effectiveness of particle repositioning maneuver (PRM) for the treatment of benign paroxysmal positional vertigo (BPPV).
Thirty one patients with BPPV of the posterior semicircular canal treated between July 1996 and June 1998 were retrospectively analyzed. Patients received a single treatment of PRM based on the hypothesis that the vertigo and nystagmus of BPPV were due to debris floating in the long arm of the posterior canal (canalithiasis). Treatment outcome was classified as cure (asymptomatic), improvement reduction of symptoms > 50%, or no change.
After a single treatment of PRM, complete remission of vertigo and nystagmus were found in 67.7% of the patients and improvement of the symptoms was found in 19.4% of the patients. The total improvement rate was 87.1%.
The particle repositioning maneuver was effective for most of the patients with benign paroxysmal positional vertigo and can be recommended as the first-line treatment modality.

回顾性分析1996年7月至1998年6月间接受治疗的31例后半规管BPPV患者。基于BPPV的眩晕和眼球震颤是由于碎片漂浮在后半规管长臂(管结石症)的假设,患者接受单次PRM治疗。治疗结果分为治愈(无症状)、症状改善(症状减轻>50%)或无变化。
单次PRM治疗后,67.7%的患者眩晕和眼球震颤完全缓解,19.4%的患者症状改善。总改善率为87.1%。
颗粒复位法对大多数良性阵发性位置性眩晕患者有效,可推荐作为一线治疗方式。
