This article offers an evaluation form to evaluate the craniomandibular pain and dysfunction patient. This form combines dental and physical therapy evaluation components in a two-page format that is quick and easy to use. By using this form physical therapists are able to determine the problem list, develop the treatment plan, and establish excellent communication between the patient, therapist, and dentist.
