A 40 year-old man with advanced pigmentary glaucoma in the left eye underwent trabeculectomy. In the immediate postoperative period, visual acuity decreased with no alterations noted on examination of the fundus or on angiofluoresceingraphy. Through optical coherence tomography, we were able to make a diagnosis of hypotonic maculopathy and treatment was immediately started.
Hypotony maculopathy is one of the most feared complications of filtering eye surgery. In some cases, optical coherence tomography can be useful to confirm its diagnosis non invasively.
