This clinical study was to improve the surgical treatment to craniomaxillofacial tissue defects.
Since 1997, eight cases with severe craniomaxillofacial defects were treated using free latissimus dorsi myocutaneous flaps. In the operation, nerve anastomosis was performed. Of the 8 cases, 7 were treated in one stage, 1 was treated in 3 steps. The craniomaxillofacial defects ranged from 10 cm x 8 cm to 30 cm x 12 cm. The flaps was 12 cm x 10 cm to 32 cm x 16 cm in size.
Postoperative follow-up for 6 months to 4 years demonstrated satisfactory results in all the cases. There was neither necrosis nor ulcer after the operation. The sensation recovery of the flap was also satisfactory.
Free transfer of the latissimus dorsi myocutaneous flap is an ideal treatment to severe craniomaxillofacial defects as it possesses the advantages of reliable blood supply, ability against infections, large size, concealed donor site, and functional restoration of sensation and movement.
本临床研究旨在改进颅颌面组织缺损的外科治疗方法。
自1997年起,采用背阔肌肌皮瓣游离移植治疗8例严重颅颌面缺损患者。术中进行了神经吻合。8例患者中,7例一期完成治疗,1例分3期治疗。颅颌面缺损范围为10 cm×8 cm至30 cm×12 cm。皮瓣大小为12 cm×10 cm至32 cm×16 cm。
术后随访6个月至4年,所有病例效果满意。术后未出现坏死及溃疡。皮瓣感觉恢复情况也令人满意。
背阔肌肌皮瓣游离移植是治疗严重颅颌面缺损的理想方法,因其具有血供可靠、抗感染能力强、面积大、供区隐蔽以及感觉和运动功能恢复等优点。
