In civilized countries blood pressure is increased in association with age. Elderly population is lately increased both in Japan and Western countries and therefore elderly hypertensive patients are expected to increase. Recently, several guidelines and statements for the treatment of hypertension, such as JNC 7, 2003 ESH/ESC, 2003 WHO/ISH, and BSH IV, are reported in Western countries. In Japan, Japanese Society of Hypertension reported guidelines for management of hypertension in 2000 (JSH 2000) and JSH 2000 is now revised and a new version was published at the end of 2004 (JSH 2004). In this article, management of hypertension (when starts the treatment, target blood pressure level, and choice of drugs, etc) in the elderly is described based on these guidelines and statements.
在文明国家,血压会随着年龄增长而升高。近年来,日本和西方国家的老年人口均有所增加,因此老年高血压患者预计也会增多。最近,西方国家报道了多项高血压治疗指南和声明,如2003年美国国家联合委员会第七次报告(JNC 7)、2003年欧洲高血压学会/欧洲心脏病学会(ESH/ESC)、2003年世界卫生组织/国际高血压学会(WHO/ISH)以及英国高血压学会第四次报告(BSH IV)。在日本,日本高血压学会于2000年发布了高血压管理指南(JSH 2000),目前JSH 2000正在修订,并于2004年底发布了新版本(JSH 2004)。本文将基于这些指南和声明,阐述老年人高血压的管理(何时开始治疗、目标血压水平以及药物选择等)。
