To report the immediate and mid-term follow-up results of the Amplatzer membranous VSD occluder for the percutaneous occlusion of the perimembranous VSD.
Percutaneous perimembranous VSD occlusion is still considered an experimental method where a variety of devices have been tested. Nowadays, more than 500 membranous Amplatzer devices have been implanted worldwide with encouraging results.
We included 6 patients (1 man and 5 women) with a mean age of 9.9 years (range, 3 to 17.5) in whom percutaneous perimembranous VSD closure was attempted.
In one of the patients, positioning of the device was not possible (intention to treat success rate, 83.3%). In the remaining five patients, there was a single defect. The VSD mean diameter with echo was 7 +/- 1.7 mm (range, 5.1 to 9) and with angio was 6.9 +/- 1 (range, 6 to 8). Mean pulmonary pressure was 20.2 +/- 7.7 mm Hg (range, 12-30) and Qp/Qs was 1.69 +/- 0.65 (range, 1.2-2.8). A single device was use in all cases. Immediate angiographic control showed complete occlusion in two patients, trivial shunt in one, and mild shunt in two. Follow-up was at least 4 months. Only one patient has residual trivial shunt, the rest of the defects are completely closed.
The special design of the Amplatzer membranous VSD occluder allows percutaneous closure of this defect in a safe and effective way, with good mid-term results. In selected cases, this is a good alternative to surgery in the treatmen of this cardiac defect.

经皮膜周部室间隔缺损封堵术仍被视为一种实验性方法,已有多种装置接受了测试。目前,全球已植入500多个膜周部Amplatzer装置,结果令人鼓舞。
我们纳入了6例患者(1例男性和5例女性),平均年龄9.9岁(范围3至17.5岁),尝试行经皮膜周部室间隔缺损封堵术。
1例患者无法进行装置定位(意向性治疗成功率为83.3%)。其余5例患者为单一缺损。超声心动图测得的室间隔缺损平均直径为7±1.7mm(范围5.1至9mm),血管造影测得的为6.9±1mm(范围6至8mm)。平均肺动脉压为20.2±7.7mmHg(范围12至30mmHg),肺循环血流量与体循环血流量之比为1.69±0.65(范围1.2至2.8)。所有病例均使用单一装置。即刻血管造影检查显示,2例患者完全封堵,1例微量分流,2例轻度分流。随访至少4个月。仅1例患者有微量残余分流,其余缺损均完全闭合。
Amplatzer膜周部室间隔缺损封堵器的特殊设计可安全有效地经皮闭合该缺损,中期效果良好。在特定病例中,这是治疗该心脏缺损的手术的良好替代方法。
