There is much controversy about the relationship between temporomandibular joint (TMJ) disorders and the skeletal structures of the lower face. It is not clear whether the disharmony of the facial skeletal structure is caused by the TMJ disorder or vice versa. The aim of this study was to determine the relationship between craniofacial skeletal structures and TMJ disorders by using lateral cephalogram measurements to examine the characteristics of the facial profile of patients with TMJ disorders.
Of 111 patients over 18 years of age from the Department of Orthodontics, Dental Hospital, Yonsei University, those showing symptoms of TMJ disorders were chosen as the experimental group (56 patients), and patients without TMJ disorders were chosen as the control group (55 patients). A lateral cephalogram of each subject was taken and traced to confirm the significance of the craniofacial measurements between the experimental group and the control group of Class I (mean ANB angle, 2.89 degrees), Class II (mean ANB angle, 6.32 degrees), Class III (mean ANB angle, -2.02 degrees) patients, who were grouped according to ANB-angle difference.
Each experimental subject with a TMJ disorder had a hyperdivergent facial profile, more lingual tilting of the maxillary incisors, and a steeper inclined occlusal plane.
There was a significant correlation between the structure of the lower face and the temporomandibular disorder.
颞下颌关节(TMJ)紊乱与下颌面部骨骼结构之间的关系存在诸多争议。目前尚不清楚面部骨骼结构的不协调是由颞下颌关节紊乱引起的,还是反之亦然。本研究的目的是通过使用头颅侧位片测量来检查颞下颌关节紊乱患者的面部轮廓特征,以确定颅面骨骼结构与颞下颌关节紊乱之间的关系。
从延世大学牙科学院正畸科选取111名18岁以上的患者,其中有颞下颌关节紊乱症状的患者被选为实验组(56例),无颞下颌关节紊乱的患者被选为对照组(55例)。对每个受试者拍摄头颅侧位片并进行描图,以确认根据ANB角差异分组的I类(平均ANB角为2.89度)、II类(平均ANB角为6.32度)、III类(平均ANB角为 -2.02度)患者实验组和对照组之间颅面测量的意义。
每个患有颞下颌关节紊乱的实验对象都有一个高角型面部轮廓,上颌切牙更多舌倾,咬合平面倾斜度更大。
下颌面部结构与颞下颌关节紊乱之间存在显著相关性。
