Immune reconstitution syndrome resulting from bacille Calmette Guerin (BCG) vaccine occurring 2 weeks to 2 months after institution of antiretroviral therapy (ART) in children has been well documented. We report the earliest onset of BCG-related immune reconstitution syndrome developing 1 week after initiation of ART.
