Obese children are predisposed to left ventricular (LV) hypertrophy and cardiac dysfunction. We evaluated ventricular function of obese children with conventional echocardiography and tissue Doppler imaging (TDI) and correlated these with fasting serum glucose, lipid and insulin levels.
Thirty obese children were examined by conventional echocardiography and TDI and compared with an age-matched control group. In the obese children, fasting serum glucose, lipid, and insulin levels were obtained.
Systolic and diastolic function of the LV was normal in obese children. LVM/ht2.7 (LVM normalized for height), relative wall thickness (RWT), and LV end-diastolic diameter were significantly greater in obese children. The early to late relaxation velocity ratio (E/A) determined by TDI for the right ventricle and interventricular septum (IVS) were significantly lower in the obese group, LVM/ht2.7 and RWT correlated with body mass index (BMI). In the obese group, IVS and LV posterior wall thickness correlated with insulin, very low density lipoprotein, and triglyceride levels. However, we failed to show this correlation when these measurements were indexed to height.
Left ventricular hypertrophy, as evidenced by increased LV mass, was present in obese children. Higher BMI, insulin, very low density lipoprotein, and triglyceride levels were associated with LV hypertrophy. TDI revealed subclinical changes in diastolic function of the right ventricle and IVS.
肥胖儿童易患左心室(LV)肥厚和心脏功能障碍。我们采用传统超声心动图和组织多普勒成像(TDI)评估肥胖儿童的心室功能,并将这些结果与空腹血糖、血脂和胰岛素水平进行关联分析。
对30名肥胖儿童进行传统超声心动图和TDI检查,并与年龄匹配的对照组进行比较。同时测定肥胖儿童的空腹血糖、血脂和胰岛素水平。
肥胖儿童左心室的收缩和舒张功能正常。肥胖儿童的左心室质量指数(LVM/ht2.7,左心室质量经身高校正)、相对室壁厚度(RWT)和左心室舒张末期内径显著更大。TDI测定的肥胖组右心室和室间隔(IVS)的舒张早期与晚期速度比值(E/A)显著更低,LVM/ht2.7和RWT与体重指数(BMI)相关。在肥胖组中,IVS和左心室后壁厚度与胰岛素、极低密度脂蛋白和甘油三酯水平相关。然而,当这些测量值经身高校正后,我们未发现这种相关性。
