The in and out flap of Pers, described in 1967, is used for the reconstruction of alar defect. After an anatomical reminder of the blood supplied of the nasolabial region, we mentioned four cases of this flap, with some little modifications of drawing and closing of donor site. The others ways of repear of alar defect are discussed.
