The institution of a family physician was introduced in Poland to improve organization and quality of primary health care. Thus, it seems important to find out how the time factor (4 years) and the organizational changes that took place during that time have affected patients' views on primary health service accessibility. The aim of the study was to compare patients' opinions on selected aspects of the access and organization of health care provided by family physicians between the years 1998 and 2002.
Two independent surveys conducted in 1998 and 2002 using face-to-face interviews with structured questionnaires. The study was carried out in a small town in Poland. The study group consisted of two samples of patients randomly selected. Altogether 1000 interviews were obtained in survey I (1998) and 1000 from another sample in survey II (2002).
The surveys carried out at a 4-year interval showed that the accessibility of family physician services improved between 1998 and 2002. This was reflected by: more common use of registration by phone and better overall evaluation of the registration system, shorter time spent in the waiting room to see a family physician, making an appointment for a definite hour, better opinion of the visit duration and more frequent use of phone consultations.
The results our study show that primary health care reform in Poland has a positive impact on the patients' opinions about access and organization of services of family physicians.
波兰引入家庭医生制度以改善初级卫生保健的组织和质量。因此,弄清楚时间因素(4年)以及在此期间发生的组织变革如何影响患者对初级卫生服务可及性的看法似乎很重要。本研究的目的是比较1998年至2002年间患者对家庭医生提供的医疗保健服务的可及性和组织方面选定内容的看法。
1998年和2002年进行了两项独立调查,采用面对面访谈和结构化问卷。研究在波兰的一个小镇进行。研究组由随机抽取的两个患者样本组成。调查I(1998年)共获得1000份访谈,调查II(2002年)从另一个样本中获得1000份访谈。
间隔4年进行的调查显示,1998年至2002年间家庭医生服务的可及性有所改善。这体现在:通过电话挂号的使用更为普遍,对挂号系统的总体评价更好,在候诊室等待看家庭医生的时间缩短,能预约到确定的时间,对就诊时长的评价更好,以及电话咨询的使用更为频繁。
我们的研究结果表明,波兰的初级卫生保健改革对患者对家庭医生服务的可及性和组织的看法产生了积极影响。
