To report 2 cases of corneal hydrops occurring in eyes with previously healthy corneal grafts.
Retrospective case series. A report of 2 presenting cases. Both patients underwent a full ophthalmic clinical examination and anterior segment optical coherence tomography (OCT).
Two patients with longstanding corneal grafts for keratoconus presented with an acute painful red eye and reduced vision. They were found to have acute hydrops with breaks in and detachments of Descemet membrane.
Keratoconic patients with longstanding corneal grafts who present acutely with corneal edema must be carefully examined not only for signs of graft rejection or failure but also for acute hydrops. Important differentiating signs may be a Descemet break or detachment and the presence of stromal edema across the donor-host corneal interface.

2例长期角膜移植治疗圆锥角膜的患者出现急性眼痛、眼红及视力下降。检查发现为急性角膜水肿,后弹力层破裂及脱离。
长期角膜移植的圆锥角膜患者若急性出现角膜水肿,不仅要仔细检查移植片排斥或失败的体征,还要检查是否为急性角膜水肿。重要的鉴别体征可能是后弹力层破裂或脱离以及供体-宿主角膜界面处基质水肿的存在。
