Giant omphalocoele in the neonate is a challenging surgical emergency and requires individualized approaches to operative repair. Post-operative complications and associated congenital anomalies still continue to exert a high morbidity and mortality in spite of intensive and improved post-operative care. This case report describes an unusual mechanism which produced anuria in a neonate immediately after operative reduction of a giant omphalocoele. In retrospect, this complication was related to the unique anatomy of the neonate and the type of closure utilized.
