An 83-year-old man presented with pain in his right groin, limping, and the ultrasound findings of an iliopectineal bursitis as the first sign of polymyalgia rheumatica. The systemic disease of polymyalgia rheumatica may begin with local signs caused by peripheral musculoskeletal inflammation. Ultrasound or magnetic resonance imaging (MRI) is appropriate in order to decide on early oral prednisolone.
