A 22-year-old pregnant woman presented with presumed immune thrombocytopenia. Postpartum, the persistence of Dohle bodies and macrothrombocytopenia led to the diagnosis of the May-Hegglin anomaly. Clinical management is described. The May-Hegglin anomaly may be mimicked by immune thrombocytopenia in pregnancy unless suspected and carefully excluded.
