Bed shortages, efficient modern diagnostic services and increasing risks associated with hospital admission, require review of conventional medical practice. Patients referred as emergencies who are walking, talking sense, eating and drinking and have normal sphincter function can usually be managed as outpatients. Widespread adoption of this practice would require more general physicians and nurse practitioners near the front door, rapid access and early review clinics and prompt diagnostic support. Education of the public, changes in medical training and reallocation of resources would reduce the need for inpatient management, providing beds for those in real need.
床位短缺、高效的现代诊断服务以及与住院相关的风险增加,都需要对传统医疗实践进行审视。那些被作为急诊转诊而来、能行走、能正常交流、能吃喝且括约肌功能正常的患者,通常可以作为门诊病人进行处理。广泛采用这种做法将需要在前门附近配备更多的全科医生和执业护士、快速就诊和早期复查诊所以及及时的诊断支持。对公众进行教育、改变医学培训以及重新分配资源,将减少住院治疗的需求,为真正有需要的人提供床位。
