Three inquiries into why people die cover maternal and child health, patient outcome and death, and people with mental illness. Cuts in death rates can be directly linked to the inquiries' findings and they have influenced policy and targets. Funding is proving and onging challenge, with one inquiry able to follow up only two of the 30 topics suggested each year.
