A case of autoimmune haemolytic anaemia due to low-titre anti-i cold agglutinin in a Nigerian is described. The patient presented with chronic haemolytic anaemia with a positive direct antiglobulin test, the cells reacting with anticomplement (anti-C3). Further tests showed that the antibody was a low titre (1 in 512) cold anti-i antibody with a wide thermal amplitude. The case reported appears to be the first case of cold haemagglutinin disease in a Nigerian.
