The new concepts of pathogenesis and pathophysiology of purulent meningitis are discussed and evaluated in logical sequence. Possible therapeutic down-modulation of the initial burst of inflammatory responses, especially with steroids (dexamethasone), is critically analyzed. Prompt and adequate antibiotic therapy remains the essential measure in the management of patients with bacterial meningitis. The individual aspects of antimicrobial therapy are described and specific recommendations are made.
