Abstract

BACKGROUND/AIMS: Determination of optimal administered sedatives during endoscopy is vital and necessary for the minimization of their side effects. This study was designed to see the effect of sedation on endoscopy induced arterial oxygen desaturation and determination of sedatives safety during gastrointestinal endoscopy.

METHODOLOGY

In this controlled clinical trial, 90 consecutive patients scheduled for diagnostic upper gastrointestinal endoscopy were categorized into 2 groups (45 in each group). The patients in the first group were sedated with midazolam 0.1 mg/kg, propofol 1 mg/kg and fentanyl 1 microg/kg intravenously, while the patients in the second group received saline as a placebo. In both groups, blood pressure, heart rate, and arterial oxygen saturation were measured before and one minute after endoscopy, and compared.

RESULTS

No significant differences were found in arterial O2 saturation (p = 0.142) and heart rate (p = 0.154) between the two groups, one minute after endoscopy. Both systolic and diastolic blood pressures were higher in non-sedated group (p < 0.001).

CONCLUSION

Concomitant administration of midazolam, propofol, and fentanyl do not cause arterial desaturation, however, it can lead to blood pressure changes one minute after the beginning of endoscopy.

背景/目的:在内镜检查期间确定最佳使用的镇静剂对于将其副作用降至最低至关重要且必不可少。本研究旨在观察镇静对内镜检查引起的动脉血氧饱和度降低的影响,并确定胃肠内镜检查期间镇静剂的安全性。

方法

在这项对照临床试验中,90例连续安排进行诊断性上消化道内镜检查的患者被分为2组(每组45例)。第一组患者静脉注射咪达唑仑0.1mg/kg、丙泊酚1mg/kg和芬太尼1μg/kg进行镇静,而第二组患者接受生理盐水作为安慰剂。两组患者在胃镜检查前和检查后1分钟测量血压、心率和动脉血氧饱和度,并进行比较。

结果

内镜检查后1分钟,两组间动脉血氧饱和度(p = 0.142)和心率(p = 0.154)无显著差异。未镇静组的收缩压和舒张压均较高(p < 0.001)。

结论

同时使用咪达唑仑、丙泊酚和芬太尼不会导致动脉血氧饱和度降低,然而,在内镜检查开始1分钟后可能会导致血压变化。
