We report a case of cardiac tamponade (CT) following a central venous catheterization with a polyethylene catheter, which perforated the myocardial wall during total parenteral nutrition (TPN). Our patient survived due to aspiration of 380 ml of the TPN solution through the catheter. The most significant clinical data was the development of a critical circulatory shock. Jugular distention was not observed nor suggestive radiological signs and the diagnosis was confirmed by radiological contrast injection through the catheter. CT is one of the most serious complications of central venous catheterization. Its occurrence is lower than 1% but it has a mortality rate of nearly 65%.
我们报告一例在经外周中心静脉置管使用聚乙烯导管进行全胃肠外营养(TPN)期间,导管穿破心肌壁导致心脏压塞(CT)的病例。通过该导管抽出380ml TPN溶液,患者得以存活。最显著的临床数据是出现严重循环性休克。未观察到颈静脉怒张,也无提示性影像学征象,通过导管注入造影剂确诊。心脏压塞是中心静脉置管最严重的并发症之一。其发生率低于1%,但死亡率接近65%。
