A 22-year-old man was found in a head-down position in a roadside gully filled with water. Circumstances and autopsy findings are discussed. A prolonged head-down position may lead to death by impairing respiration and causing circulatory dysregulation. In the presented case, the victim drowned in a tight shaft in which he had no chance to free himself.
