A decade ago, the Institute of Medicine released its landmark report, To Err Is Human, and brought a sharp focus among the media, policymakers and providers to the issue of patient safety. H&HN asked five leading voices representing diverse health care constituencies to discuss how far the United States has come in improving safety and what steps need to be taken now to maintain--and build--momentum.
