Surgical resection remains the treatment of choice for pulmonary arteriovenous malformation but removes some normal lung parenchyma. This study aimed to evaluate the effect and safety of the lung-saving procedure of fistulectomy as an alternative to lung resection.
From July 2003 to July 2008, 6 selected patients with pulmonary arteriovenous malformations underwent fistulectomies. Among them, 1 patient underwent emergency operation and 2 underwent bilateral operations. One patient received postoperative embolotherapy.
No hospital deaths or postoperative morbidity occurred. PaO2 increased significantly after operation. All patients were free of symptoms and hypoxia during a follow-up for 9 months to 5 years.
Fistulectomy is a safe and effective procedure for patients with pulmonary arteriovenous malformation and may be an alternative to lung resection.
手术切除仍然是治疗肺动静脉畸形的首选方法，但会切除部分正常肺组织。本研究旨在评估瘘管切除术作为肺切除术替代方法的效果和安全性。
从 2003 年 7 月至 2008 年 7 月，6 名经选择的肺动静脉畸形患者接受了瘘管切除术。其中，1 例患者行急诊手术，2 例患者行双侧手术。1 例患者接受了术后栓塞治疗。
无院内死亡或术后并发症。术后 PaO2 显著升高。所有患者在 9 个月至 5 年的随访期间均无症状和缺氧。
瘘管切除术是一种安全有效的肺动静脉畸形治疗方法，可能是肺切除术的替代方法。
