Abstract

PURPOSE

To evaluate the success of zygomatic plate-screw anchorage and to define the key points that help to improve the success of this system.

MATERIALS AND METHODS

A total of 74 zygomatic plate-screw anchors were applied to 37 patients from 2 groups receiving orthodontic treatment. The first group consisted of 19 patients, and the zygoma anchors were applied bilaterally to distalize the maxillary buccal segment. The second group consisted of 18 patients, and the zygoma anchors were applied bilaterally to stabilize the maxillary molars during maxillary canine retraction. The orthodontic force was applied 1 week after the insertion of the plates. In the first group, 450 g of direct force and in the second group 150 g of indirect force were applied to the zygomatic plates. The success rate of the zygomatic plate-screw anchorage system was evaluated.

RESULTS

One plate was lost and the others remained stable all through the orthodontic treatment. Mild gingival inflammation was observed in 1 patient (2 plates), and pus formation was detected in 1 patient (2 plates). One plate was covered because of mucosal hypertrophy.

CONCLUSION

Zygomatic plate-screw anchorage system is a reliable technique to obtain orthodontic anchorage and may eliminate the need for extraoral force. However, the surgical insertion technique, position of the plates, and oral hygiene status of the patients certainly influence the success of the system.

目的

评估颧板螺钉支抗的成功率,并确定有助于提高该系统成功率的关键点。

材料与方法

对两组接受正畸治疗的37例患者共应用74枚颧板螺钉支抗。第一组由19例患者组成,双侧应用颧突支抗使上颌颊侧段远中移动。第二组由18例患者组成,双侧应用颧突支抗在牵引上颌尖牙期间稳定上颌磨牙。在植入支抗板1周后施加正畸力。第一组对颧板施加450g的直接力,第二组施加150g的间接力。评估颧板螺钉支抗系统的成功率。

结果

在整个正畸治疗过程中,1块支抗板脱落,其他支抗板保持稳定。1例患者(2块支抗板)出现轻度牙龈炎症,1例患者(2块支抗板)检测到有脓液形成。1块支抗板因黏膜肥大而被覆盖。

结论

颧板螺钉支抗系统是获得正畸支抗的可靠技术,可能无需口外力。然而,手术植入技术、支抗板位置以及患者的口腔卫生状况肯定会影响该系统的成功率。
