Abstract

BACKGROUND

Rupture of pregnant rudimentary horn in a pseudounicornuate uterus is usually resulting in severe and dramatic haemoperitoneum at the beginning of the second trimester of gestation. Advanced abdominal pregnancy due to horn rudimentary uterine rupture with delivery of a viable fetus is exceptional.

AIM

To analyse obstetrical entailments, diagnosis and current concepts of management of advanced abdominal pregnancy secondary to rudimentary horn rupture.

CASE

An asymptomatic ruptured rudimentary horn pregnancy in a 31-year-old, second gravida, is reported. She was referred to our maternity for anhidramnios at estimated gestational age of 30 weeks. An advanced abdominal pregnancy was diagnosed with sonographic features suggestive of horn rudimentary uterine rupture. Elective laparotomy was performed and a healthy infant was delivered. Excision of the rudimentary horn was done and an uneventful recovery followed.

CONCLUSION

An unusual sonographic appearance of the placenta with anhidramnios must first lead to consider the diagnosis of advanced abdominal pregnancy in time to save the surgeon from an unpleasant and dangerous surprise and to increase the chance of materno-fetal survival.

背景

残角子宫妊娠破裂通常发生在妊娠中期开始时,会导致严重且急剧的腹腔内出血。因残角子宫破裂导致晚期腹腔妊娠并娩出存活胎儿的情况极为罕见。

目的

分析残角子宫破裂继发晚期腹腔妊娠的产科并发症、诊断及当前的处理观念。

病例

报道一名31岁、孕2产0的无症状残角子宫妊娠破裂患者。她因孕30周估计羊水过少被转诊至我院产科。经超声检查诊断为晚期腹腔妊娠,其特征提示残角子宫破裂。行择期剖腹手术,娩出一名健康婴儿。切除残角子宫后,患者恢复顺利。

结论

胎盘出现异常超声表现且伴有羊水过少时,必须首先考虑晚期腹腔妊娠的诊断,以便及时避免给外科医生带来意外和危险,并增加母婴存活几率。
