Abstract

OBJECTIVE

To evaluate the role of real-time polymerase chain reaction (real-time PCR) assay combined with serum galactomannan (GM) detection in the diagnosis of invasive aspergillosis (IA) with hematological malignancies or hematopoietic stem cell transplantation.

METHODS

Eighty patients were enrolled. PCR and GM were performed simultaneously on 205 serum samples. The agreement between these two assays and the diagnostic value of the combination were analyzed.

RESULTS

There were 5 proven IA, 20 probable IA, 34 possible IA and 21 non-IA. Agreement between real-time PCR and GM was 77.5%. When both positive, the positive predictive value could reach 100%; when both negative, the negative predictive value could reach 90.9%.

CONCLUSION

PCR agrees well with GM. When these two assays are combined, invasive aspergillosis can be diagnosed more accurately.

目的

评估实时聚合酶链反应(real-time PCR)检测联合血清半乳甘露聚糖(GM)检测在血液系统恶性肿瘤或造血干细胞移植患者侵袭性曲霉病(IA)诊断中的作用。

方法

纳入80例患者。对205份血清样本同时进行PCR和GM检测。分析这两种检测方法之间的一致性以及联合检测的诊断价值。

结果

确诊IA患者5例,拟诊IA患者20例,可能IA患者34例,非IA患者21例。实时PCR与GM检测的一致性为77.5%。两者均为阳性时,阳性预测值可达100%;两者均为阴性时,阴性预测值可达90.9%。

结论

PCR与GM检测结果一致性良好。两种检测方法联合应用时,可更准确地诊断侵袭性曲霉病。
