Abstract

The number of home care patients with intractable neurological diseases has been increasing. However, a doctor who specializes in neurology is not always involved in the continuing (follow-up) home care. From the perspective of a doctor who is a visiting physician, but is not specialized in neurology, this study considers the issues related to an introduction and cooperative efforts in home care with the aim of smoothly implementing the continuing (follow-up) medical care. Based on case experiences, it is believed that communication is important for building of the relationship of trust with patients, caregivers, and family members as well as a smooth cooperation with staff members in the team who are involved in continuation of medical care. Furthermore, it is thought that communication and cooperation through the exchange and sharing of information among doctors, who are specialists in neurology and visiting physicians, would result in both safer and more relieved feelings for patients, and are necessary for maintaining a better medical environment for home care patients. Based on the above, it is believed that communication is the key to the success of both smooth cooperative relationships among staff members in the team and a continuation of successful medical care for home care patients with intractable neurological diseases.

患有难治性神经系统疾病的家庭护理患者数量一直在增加。然而,专门从事神经学的医生并不总是参与持续的(后续的)家庭护理。从作为上门医生但并非神经学专科医生的角度来看,本研究考虑了在家庭护理中引入和合作的相关问题,目的是顺利实施持续的(后续的)医疗护理。基于案例经验,人们认为沟通对于与患者、护理人员和家庭成员建立信任关系以及与参与医疗护理延续的团队工作人员顺利合作非常重要。此外,人们认为通过神经学专科医生和上门医生之间的信息交流和共享进行沟通与合作,将为患者带来更安全和更安心的感受,并且对于为难治性神经系统疾病的家庭护理患者维持更好的医疗环境是必要的。基于以上所述,人们认为沟通是团队工作人员之间顺利合作关系以及为难治性神经系统疾病的家庭护理患者成功延续医疗护理的关键。
