To attain optimal therapeutic effects, a drug must reach its target structure in sufficiently high concentrations for a sufficiently long time. Lacking or insufficient response may be due to insufficient drug concentrations. It may be caused either by non compliance of the patient or by reduced absorption or ultra rapid metabolism of the drug. To identify reasons underlying suboptimal plasma concentrations, therapeutic drug monitoring (TDM) and, in special cases, genotyping are most helpful. When applied appropriately, they accelerate the response to acute drug treatment and prevent relapses under chronic treatment.
为达到最佳治疗效果,药物必须在足够长的时间内以足够高的浓度到达其靶结构。疗效欠佳或疗效不足可能是由于药物浓度不足所致。这可能是由患者不依从、药物吸收减少或药物代谢极快引起的。为确定血浆浓度未达最佳水平的潜在原因,治疗药物监测(TDM)以及在特殊情况下进行基因分型最为有用。若应用得当,它们可加快急性药物治疗的反应速度,并防止慢性治疗期间病情复发。
