Examination of 5366 dry mandibles at the Smithsonian Institution revealed three cases of unilateral lingual defects superior to the mandibular foramen. The differential diagnosis includes a neuroma, neurolemmoma, hemangioma, or aneurysm of the inferior alveolar artery. The clinical significance of a vascular neoplasm or aneurysm would be the likelihood of increasing the risk of injecting the anesthetic solution directly into a blood vessel.
