Vallecular cyst is one of the unusual causes of respiratory distress in newborns. Recently we encountered a case of 22-day-old male infant admitted due to respiratory distress, stridor and poor feeding. A lateral view of neck X-ray examinations demonstrated a soft tissue mass in the hypopharyngeal area. On direct laryngoscopic examination, a cystic mass was noted on the left side surface of the epiglottis. The cyst appeared to be involving the vallecula compressing the tongue base and epiglottis. The cyst was partially excised and marsupialized. It was proved to be a vallecular cyst by pathological examination.
