Abstract

BACKGROUND/AIMS: Acute pancreatitis is a severe and frequently a life-threatening disease which can lead to pancreatic necrosis, acute lung injury, SIRS and MODS. The pathophysiologic pattern of acute pancreatitis is being investigated, when guidelines for treatment are being modified often and novel therapeutic strategies have failed to show real benefit in clinical practice.

METHODOLOGY

The present paper reviews the role of nuclear factors NF-kappaB and AP-1, TNFalpha, and TLR-4 in acute pancreatitis.

CONCLUSION

Understanding the inflammatory mediators expression, regulation of apoptosis and dissecting the stress activating signaling pathways in acute pancreatitis, is of paramount importance in order to achieve in the near future adequate therapeutic interventions.

摘要

背景/目的:急性胰腺炎是一种严重且常危及生命的疾病,可导致胰腺坏死、急性肺损伤、全身炎症反应综合征(SIRS)和多器官功能障碍综合征(MODS)。急性胰腺炎的病理生理模式正在研究中,此时治疗指南经常修改,而新的治疗策略在临床实践中未能显示出实际益处。

方法

本文综述了核因子NF-κB和AP-1、肿瘤坏死因子α(TNFα)以及Toll样受体4(TLR-4)在急性胰腺炎中的作用。

结论

了解急性胰腺炎中炎症介质的表达、细胞凋亡的调控以及剖析应激激活信号通路,对于在不久的将来实现充分的治疗干预至关重要。
