Six patients with gallstone ileus were under observation. The diagnosis was not established before the operation. Surgery was undertaken for acute ileus without identification of the cause. Stones in the intestinal lumen were an unexpected operative finding. The operation of choice was a minimal intervention for the restoration of intestinal continuity (enterolithotomy, resection of the intestine whenever necessary). Five patients recovered, one died.
