Based on a case report the rare occurrence of tracheal damage by a Carlens tube is described. A tension pneumothorax in immediate connection with intubation difficulties can be an indication of tracheal damage. The surgical treatment and the reconstruction of the tracheal lesion is described. The aim of this case report was to show that a recovery is possible through early recognition of such a life-threatening complication.
