Best practice model for immunization: one home care agency's experience providing H1N1 and seasonal vaccine.

This article may serve as a framework for school-based immunization initiatives. During the fall of 2009, VNA Home Health & Hospice, in close collaboration with the Maine Centers for Disease Control, organized and performed over 113 school-based clinics. These efforts helped Maine achieve a very high percentage of vaccination rates in the state's school-age population.
