The process of applying for National Committee for Quality Assurance recognition as a Physician Practice Connections--Patient-Centered Medical Home (PPC-PCMH) can enrich even those practices already solidly committed to providing patient-centric care based on evidence-based medical guidelines. The goal is to leverage information technology to transform both operational and patient care procedures. But even with robust technology, PCMH certification requires team commitment and an unremitting big-picture focus.
