We report the case of a female aged 67 years suffering a Waldenström's macroglobulinemia which began with arthritis at the knee and pleural effusion. In both locations the anatomopathological examination showed infiltration by typical lympho-plasmatic cells. Other possible etiologies such as amyloid, infection or arthritis produced by microcrystals were ruled out. The relevant finding in this case was the presence of infiltrative reaction at the synovial tissue which has been rarely reported in the literature. We review the incidence of articular manifestations in cases of Waldenström's macroglobulinemia and the possible pathogenetic mechanisms involved.
我们报告了一例67岁女性患华氏巨球蛋白血症的病例，该病始于膝关节关节炎和胸腔积液。在这两个部位，解剖病理学检查均显示有典型淋巴细胞-浆细胞浸润。其他可能的病因，如淀粉样变、感染或微晶引起的关节炎，均被排除。该病例的相关发现是滑膜组织存在浸润性反应，这在文献中鲜有报道。我们回顾了华氏巨球蛋白血症病例中关节表现的发生率以及可能涉及的发病机制。
