To explore the relationships between serum levels of adiponectin, leptin, TNF-alpha and CRP (c-reactive protein) versus insulin resistance in early pregnancy.
A total of 342 pregnant women delivering at our hospital were recruited from October 2007 to July 2008. Their clinical data were collected and analyzed. According to the 50-g 1-hour oral glucose screening test, they were divided into 3 groups: gestational diabetes, gestational impaired glucose tolerance and normal pregnancy. The serum levels of adiponectin, leptin, TNF-alpha and CRP were determined by ELISA (enzyme-linked immunosorbent assay) and multivariate correlation analysis was used to analyze their relationships with the indices of insulin resistance.
Among them, there were gestational diabetes (n = 13) and gestational impaired glucose tolerance (n = 19). Thirty-five normal pregnant women were selected as the control group. The result of analysis of variance indicted that adiponectin, leptin and TNF-alpha had significant statistical differences among three groups. Through the multivariate correlation analysis, adiponectin had a adverse correlation with the indices of insulin resistance. Leptin, CRP, TNF-alpha and BMI (body mass index) had positive correlations with the indices of insulin resistance.
Adiponectin, leptin, TNF-alpha and CRP are correlated with the indices of insulin resistance. And TNF-alpha may become an important predictor of gestational diabetes in early pregnancy.
探讨孕早期血清脂联素、瘦素、肿瘤坏死因子-α(TNF-α)和C反应蛋白(CRP)水平与胰岛素抵抗之间的关系。
选取2007年10月至2008年7月在我院分娩的342例孕妇,收集并分析其临床资料。根据50克1小时口服葡萄糖筛查试验,将她们分为3组:妊娠期糖尿病组、妊娠期糖耐量受损组和正常妊娠组。采用酶联免疫吸附测定法(ELISA)测定血清脂联素、瘦素、TNF-α和CRP水平,并运用多因素相关性分析来分析它们与胰岛素抵抗指标之间的关系。
其中,妊娠期糖尿病患者13例,妊娠期糖耐量受损患者19例。选取35例正常孕妇作为对照组。方差分析结果表明,脂联素、瘦素和TNF-α在三组之间存在显著统计学差异。通过多因素相关性分析,脂联素与胰岛素抵抗指标呈负相关。瘦素、CRP、TNF-α和体重指数(BMI)与胰岛素抵抗指标呈正相关。
脂联素、瘦素、TNF-α和CRP与胰岛素抵抗指标相关。并且TNF-α可能成为孕早期妊娠期糖尿病的重要预测指标。
