Mesothelioma is a malignant tumour of the serous membranes that principally affects the pleura. Peritoneal, pericardial and tunica vaginalis mesothelioma are very rare.
We report the case of a 65-year-old male with malignant mesothelioma of the tunica vaginalis (MTV). He presented with several local recurrences and, five years after the initial surgery, with pulmonary nodules and a pleural effusion. Pleural biopsies confirmed epithelioid mesothelioma. A diagnosis of pleuro-pulmonary metastases from previous malignant MTV was made.
Malignant MTV is a rare and aggressive tumor with frequent local recurrences and, rarely, visceral metastases. This case report emphasizes the difficulties of the differential diagnosis between pleural mesothelioma and pleural metastases from MTV. The lack of any treatment for metastatic malignant MTV is discussed.
间皮瘤是一种主要累及胸膜的浆膜恶性肿瘤。腹膜、心包和睾丸鞘膜间皮瘤非常罕见。
我们报告一例65岁男性睾丸鞘膜恶性间皮瘤（MTV）病例。他出现了几次局部复发，在初次手术后五年，出现了肺结节和胸腔积液。胸膜活检证实为上皮样间皮瘤。诊断为既往恶性MTV的胸膜肺转移。
恶性MTV是一种罕见且侵袭性强的肿瘤，常有局部复发，很少有内脏转移。本病例报告强调了胸膜间皮瘤与MTV胸膜转移之间鉴别诊断的困难。讨论了转移性恶性MTV缺乏有效治疗方法的问题。
