Chronic presacral sinus at the anastomosis after restorative proctocolectomy and ileal pouch-anal anastomosis (IPAA) has been difficult to manage and results in a high rate of pouch failure. The aim was to present a novel technique with a combining endoscopic doxycycline injection and needle knife therapy for a sinus at the ileoanal anastomosis.
We applied repeat Doppler ultrasound-guided endoscopic needle knife therapy to open the orifice of the anastomotic sinus followed by topical injection of doxycycline.
After six sessions of the endoscopic therapy, the refractory sinus was completely healed.
The Doppler ultrasound guided endoscopy therapy may provide an alternative treatment option for the patients with anastomotic sinus.
直肠结肠切除术和回肠储袋肛管吻合术（IPAA）后吻合口前方慢性骶前窦难以处理，导致储袋失败率高。本研究旨在介绍一种新的技术，即联合内镜强力霉素注射和针刀治疗回肠肛管吻合口窦道。
我们应用重复经直肠多普勒超声引导内镜下针刀治疗，以打开吻合口窦道的开口，然后局部注射强力霉素。
