Concave lower lateral cartilages are rare but impressive findings that can best be exposed by open rhinoplasty. Surgical treatment takes advantage of the existing vaulting of the cartilage. A 180 degrees rotation flap is recommended for unilateral cases and a cross-over rotation flap for bilateral deformities. The concave form of lateral crura is probably more frequent than suspected, and it is therefore necessary to identify this lesion and to be familiar with techniques for its correction.
