Topical and systemic antibiotic therapy is common in dermatology, yet it is hard to find a rationale for a particular route in some diseases. Any rational choice needs to take efficacy, cost, and the likelihood of antibiotic resistance into account. Aspects of these factors are discussed in relation to the therapy of infectious and noninfectious bacterial skin diseases.
