Chemotherapy-related hypersensitivity reaction in Japanese patients with gynecologic malignancy.

PURPOSE OF INVESTIGATION
Chemotherapy-related hypersensitivity reaction seems to be problematic in the safe management of chemotherapy. In this study we investigated chemotherapy-related hypersensitivity reaction in patients with gynecologic malignancy.
METHODS
Between January 2009 and December 2010, we examined hypersensitivity reaction (> or = grade2) using the Common Terminology Criteria for Adverse Events (CTCAE) v.4.0. We analyzed the incidence, clinical features, management, and outcome.
RESULTS
We administered over 1,057 infusions (24 regimens) to 205 patients. We found a total of four hypersensitivity reactions (> or = grade 2) cases (carboplatin: 2; nedaplatin: 1; docetaxel: 1). Signs and symptoms were varied. In two cases, the same regimen was rechallenged by using anti-allergic drugs. The docetaxel case was successful. The carboplatin case was not successful.
CONCLUSION
Chemotherapy-related hypersensitivity reaction (> or = grade2) does not occur frequently. In the case of platinum, especially, carboplatin, re-administering after hypersensitivity reaction should be done carefully though platinum is a key drug in patients with gynecologic malignancies.
研究目的
化疗相关的超敏反应似乎是化疗安全管理中的一个问题。在本研究中，我们调查了妇科恶性肿瘤患者中化疗相关的超敏反应。
方法
在2009年1月至2010年12月期间，我们使用不良事件通用术语标准（CTCAE）v.4.0检查超敏反应（≥2级）。我们分析了发病率、临床特征、管理和结果。
结果
我们对205例患者进行了超过1057次输注（24种方案）。我们共发现4例超敏反应（≥2级）病例（卡铂：2例；奈达铂：1例；多西他赛：1例）。体征和症状各不相同。在2例病例中，使用抗过敏药物对相同方案进行了再次挑战。多西他赛病例成功。卡铂病例未成功。
结论
化疗相关的超敏反应（≥2级）并不经常发生。特别是对于铂类药物，尤其是卡铂，尽管铂类是妇科恶性肿瘤患者的关键药物，但在超敏反应后再次给药时应谨慎。
