Abstract

OBJECTIVE

To assess the short-term efficacy of Semont maneuver for unilateral posterior semicircular canal benign paroxysmal positional vertigo (PC-BPPV).

METHODS

The short-term efficacy of Semont maneuver was compared with a sham procedure in 128 patients with PC-BPPV by using a double-blinded prospective randomized controlled trial in the setting of a neurologic outpatient clinic in China. Analysis was based on the symptomatic status and the Dix-Hallpike maneuver on the fourth day. At this time, patients in both groups with a still positive Dix-Hallpike test were treated with Semont maneuver, and outcome was again assessed on the seventh day.

RESULTS

On the fourth day, 55 (84.62%) of 65 patients in the treatment group showed complete resolution of symptoms compared with 9 (14.29%) of 63 patients in the control group. (p < 0.001) 59 (90.77%) of 65 and 52 (82.54%) of 63 patients reported resolution of symptoms in the treatment and original sham group, respectively (p = 0.17).

CONCLUSION

Semont maneuver is shown to resolve PC-BPPV effectively, and patients should receive Semont maneuver once the PC-BPPV diagnosis was confirmed.

目的

评估 Semont 手法治疗单侧后半规管良性阵发性位置性眩晕(PC-BPPV)的短期疗效。

方法

在中国一家神经病学门诊的背景下,采用双盲前瞻性随机对照试验,比较 Semont 手法与假手术治疗 128 例 PC-BPPV 患者的短期疗效。分析基于症状状态和第 4 天的 Dix-Hallpike 试验。此时,两组中 Dix-Hallpike 试验仍为阳性的患者均接受 Semont 手法治疗,第 7 天再次评估疗效。

结果

第 4 天,治疗组 65 例患者中 55 例(84.62%)症状完全缓解,而对照组 63 例患者中仅 9 例(14.29%)(p<0.001)。治疗组和原始假手术组分别有 59 例(90.77%)和 52 例(82.54%)患者报告症状缓解(p=0.17)。

结论

Semont 手法可有效治疗 PC-BPPV,一旦确诊,患者应接受 Semont 手法治疗。
