It took a while to get started, but UConn Health Center/John Dempsey Hospital in Farmington, CT, is now meeting monthly with dozens of post-acute providers to work on improving transitions. Providers exchange ideas for better patient care. Educational materials have been standardized throughout the continuum. Based on input from other providers, the hospital is improving the discharge materials.
