The aim of the current study was to determine the diagnostic accuracy of whole-body 18F-fluoro-2-deoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) in detecting carcinoma of unknown primary (CUP).
A total of 7,636 patients were investigated by FDG-PET/CT examinations at our Institution. We retrospectively evaluated the file records of 432 patients who were referred to FDG PET/CT imaging with a diagnosis of cancer of unknown primary, and included 316 of the patients with histopathologic verification at the final diagnosis. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy were calculated. The Kaplan-Meier test was used for survival analysis. Median survival rate was calculated to evaluate the prognostic value of the FDG-PET/ CT findings.
In the search for a primary, FDG-PET/CT findings correctly diagnosed lesions as the site of the primary true positive in 238 cases, 10 findings diagnosed no site of a primary and none was subsequently proven true negative, 12 diagnoses were false positive and 56 were false negative. The sensitivity of FDG-PET/CT is 81% and the specificity 45%. Positive predictive value, negative predictive value and diagnostic accuracy were 95%, 15% and 78%, respectively.
Whole-body FDG-PET/CT imaging is proven to be useful method in the search for the primary focus and metastases in patients with cancer of unknown primary.
本研究旨在确定全身 18F-氟代-2-脱氧葡萄糖正电子发射断层扫描/计算机断层扫描(FDG-PET/CT)在诊断不明原发灶癌(CUP)中的诊断准确性。
在我院,共有 7636 例患者接受了 FDG-PET/CT 检查。我们回顾性评估了 432 例被诊断为癌症不明原发灶并接受 FDG PET/CT 成像检查的患者的档案记录,其中 316 例患者在最终诊断时进行了组织病理学验证。计算了灵敏度、特异性、阳性预测值、阴性预测值和准确性。采用 Kaplan-Meier 检验进行生存分析。计算中位生存率以评估 FDG-PET/CT 结果的预后价值。
在寻找原发灶时,FDG-PET/CT 检查正确诊断出 238 例为原发灶的病变(真阳性),10 例诊断为无原发灶(真阴性),12 例诊断为假阳性,56 例诊断为假阴性。FDG-PET/CT 的灵敏度为 81%,特异性为 45%。阳性预测值、阴性预测值和诊断准确性分别为 95%、15%和 78%。
全身 FDG-PET/CT 成像在寻找癌症不明原发灶患者的原发灶和转移灶方面是一种有用的方法。
