Effect of a trunk-targeted intervention using vibration on posture and gait in children with spastic type cerebral palsy: a randomized control trial.

Abstract

AIM

This study aimed to determine whether strengthening trunk muscles using vibration can improve posture and gait in children with spastic-type cerebral palsy (STCP).

METHODS

A total of 27 children (6-13 years) participated in a single-blinded pre-post crossover experimental trial. The 1-Minute Walk Test, 2D-posturography, ultrasound imaging and sit-ups in one minute were used to assess effect on gait, posture, resting abdominal muscle thickness and functional strength.

RESULTS

Significant increase in distance walked (p < 0.001), more upright posture, an increase in sit-ups executed (p < 0.001) and an increase in resting thicknesses of all the four abdominal muscles--transversus abdominis (p = 0.047), obliquus internus (p = 0.003), obliquus externus (p = 0.023) and the rectus abdominis (p = 0.001) was recorded. Strength and posture were maintained at 4-weeks post-intervention.

CONCLUSION

A trunk-targeted intervention using vibration can improve posture and gait in children with STCP without any known side effects. It is recommended that vibration and specific trunk strengthening is included in training or rehabilitation programmes. Effects of vibration on force generation and spasticity need further investigation.

目的

本研究旨在确定使用振动强化躯干肌肉是否能改善痉挛型脑瘫(STCP)患儿的姿势和步态。

方法

共有27名6至13岁的儿童参与了一项单盲前后交叉实验性试验。采用1分钟步行测试、二维姿势描记法、超声成像和1分钟仰卧起坐来评估对步态、姿势、静息时腹部肌肉厚度和功能力量的影响。

结果

步行距离显著增加(p < 0.001),姿势更挺直,仰卧起坐次数增加(p < 0.001),并且记录到所有四块腹部肌肉——腹横肌(p = 0.047)、腹内斜肌(p = 0.003)、腹外斜肌(p = 0.023)和腹直肌(p = 0.001)的静息厚度增加。干预后4周时力量和姿势得以维持。

结论
