An extension deficit after ACL reconstruction is disabling for the patient. As a result of intensive physiotherapy, the extension deficit can even increase and may be painful. Arthroscopic follow-up of 121 patients with problems after ACL reconstruction revealed hyperplasia of the transplant in 30 cases. Repetitive trauma due to the "notch" creates bleeding and finally the appearance of big tumors sitting on the surface of the transplant. The extension deficit is the result of those tumors. The original cause was malalignment of the transplant. Arthroscopic shaving of the transplant (including notch plasty) proved to be very efficient. Full extension was achieved after this rather simple procedure.
前交叉韧带重建术后出现伸直受限会给患者带来不便。由于强化物理治疗，伸直受限甚至可能加重，且可能伴有疼痛。对121例前交叉韧带重建术后出现问题的患者进行关节镜随访发现，30例存在移植组织增生。“髁间切迹”处的反复创伤导致出血，最终在移植组织表面形成大的肿物。伸直受限就是这些肿物造成的。其根本原因是移植组织排列不齐。关节镜下对移植组织进行修整（包括髁间切迹成形术）被证明非常有效。经过这个相对简单的手术，患者实现了完全伸直。
