Abstract

BACKGROUND

This study attempted to identify the role of combined conventional and novel echocardiographic techniques in evaluation of left atrial (LA) function to predict the postoperative atrial fibrillation (POAF).

METHODS

In this cross-sectional study, subjects with sinus rhythm who were candidates for CABG were enrolled. Preoperative LA function was evaluated by conventional echocardiography and 2-dimensional strain imaging based-velocity vector imaging (VVI). VVI-derived systolic peak positive, early and late diastolic strain rate were measured. Using tissue Doppler study, systolic peak velocities (Sm), early diastole (Em), atrial systole (Am) and AEMi were also measured. The patients were observed during their hospital stay. The primary endpoint of the study was postoperative AF lasting > 5 minutes.

RESULTS

POAF occurred in 12.7% of patients. Age, LA volume index (LAVI), LA area, LA empting fraction (LAeF) and cardiopulmonary bypass time were found to be the independent predictors of POAF. TDI-derived velocities were similar in study groups. LA systolic strain rate (SRs) and early diastolic strain rate were impaired in patients with no significant difference between those who developed POAF and those who did not.

CONCLUSIONS

Conventional echocardiography could be used as a feasible method for evaluating subclinical atrial dysfunction in patients undergoing CABG and the use of VVI- based 2-dimensional strain imaging may need further studies.

本研究旨在探讨联合应用传统及新型超声心动图技术评估左心房(LA)功能在预测术后心房颤动(POAF)中的作用。

方法

本横断面研究纳入了窦性心律且拟行冠状动脉旁路移植术(CABG)的患者。采用传统超声心动图和二维应变成像的速度向量成像(VVI)评估术前 LA 功能。测量 VVI 衍生的收缩期峰值正向、早期和晚期舒张应变率。采用组织多普勒研究测量收缩期峰值速度(Sm)、早期舒张期(Em)、心房收缩期(Am)和 AEmi。患者在住院期间接受观察。本研究的主要终点为持续>5 分钟的术后 AF。

结果

12.7%的患者发生 POAF。年龄、左心房容积指数(LAVI)、左心房面积、LA 排空分数(LAeF)和体外循环时间是 POAF 的独立预测因素。TDI 衍生速度在研究组之间相似。发生 POAF 与未发生 POAF 的患者之间 LA 收缩期应变率(SRs)和早期舒张期应变率无显著差异。

结论

传统超声心动图可作为评估 CABG 患者亚临床心房功能障碍的可行方法,而 VVI 基础二维应变成像的应用可能需要进一步研究。
