A 20 year-old healthy patient consulted due to acute loss vision in his left eye. A fundus examination showed a macular alteration compatible with unifocal helioid choroiditis, characterized by being an atypical inflammatory yellow-white, round, single lesion of approximately an optic disc in diameter. The etiology study detected low Antitoxoplasma gondii Ig G (immunoglobulin) titers.
The non-specificity of the serology and the atypical characteristics of the lesion is a limitation in the diagnosis. The Goldmann-Witmer coefficient may be useful in the diagnosis of atypical lesions, by comparing the concentration of IgG from the serum and aqueous humor.
一名20岁健康患者因左眼急性视力丧失前来就诊。眼底检查显示黄斑病变，符合单灶性脉络膜视网膜炎，其特征为非典型炎症性黄白色圆形单发病变，直径约为一个视盘大小。病因学研究检测到抗弓形虫IgG（免疫球蛋白）滴度较低。
血清学的非特异性和病变的非典型特征是诊断的局限性。通过比较血清和房水中IgG的浓度，戈德曼-维特默系数可能有助于非典型病变的诊断。
